The “Heart of Nashville” project, funded through the Metro Nashville Public Health Department and lead by NashvilleHealth, brings together nonprofits, healthcare providers, and community partners to explore how we can collectively improve health outcomes across Nashville. By aligning efforts and sharing insights, this initiative is working to address the root causes of health disparities and create a more connected, effective system of care. A key part of this work includes understanding the broader impact of chronic disease, not only on individuals, but on the economic health of our entire community.
A vital partner in this effort is The Sycamore Institute, which serves as the process evaluator for the Heart of Nashville initiative. From the beginning, their role has been to document progress, capture lessons learned, and help partners better understand what’s working. By collecting and analyzing collaboration data across the initiative, they ensure that insights are not lost, but instead used to continuously improve outcomes. Their work also helps position the project for future growth by making it replicable in other communities.
